In Fujisawa, high school student Sakuta Azusagawa and his girlfriend and senior student Mai Sakurajima are having a great time together. However, their peaceful life was broken by the sudden appearance of Sakuta’s first love, Makinohara Shoko. While going to the hospital with his sister, Sakuta discovers two versions of Shoko: one from middle school and an older version from the future. When little Shoko has a heart condition and needs a transplant, Sakuta and Mai look for a way to help her. Sakuta learns that her heart was the donor for adult Shoko’s successful transplant, prompting her to travel back in time to save his life.
In a heart-wrenching turn, Sakuta decides to sacrifice himself, but Mai steps in, saves him and becomes Shoko’s new heart donor. Mai’s death deeply affected Sakuta, and the adult Shoko gave Sakuta a chance to rewrite history. He traveled back in time to prevent tragic events from happening.
