While Sara and Jess aren’t necessarily creepy in their own behavior, the way that the spirits in the 2016 ghost horror movie manipulate the twins and plays off of the fact they’re identical definitely is. As far as pace is concerned, The Forest is a slow burner among the best horror movies with twins in go, and it leans more into building a slow dread than jump cares. The films from British studio Hammer Horror are incredibly influential, and dominated the horror movie market in the mid 20th century. One of the great horror movies with twins in from the studio was 1971’s Twins of Evil, which starred sisters Mary and Madeline Collinson as Maria and Frieda Gellhorn. The fact that Twins of Evil was also known as Twins of Dracula gives away the plot and tone. Vampires feature heavily, and the twins are central to the story.
As far as the tone goes, Twins of Evil contains everything viewers would expect from a 1970s horror film. There are some incredible sequences, and the nostalgia factor is, like almost every release from Hammer Horror, incredibly strong. The best evil twin moments come when Frieda begins giving into the temptations of vampirism, and the performances of the Collinson sisters and Peter Cushing of Star Wars fame more than make this underrated gem worth visiting.

2023’s Tin & Tina is a religious horror movie from director Rubin Stein, and it’s laced with many disturbing psychological elements that work incredibly well for building up slow, atmospheric dread. The creepy twins in Tin & Tina are played by child actors Carlos Gonzales Morollon and Anastasia Russo, and as Tin and Tina they are incredibly unsettling.
The plot of Tin & Tina sees the titular twins adopted by Lola (Milena Smit) and Adolfo (Jaimes Lorente). The siblings are incredibly religiously devout, but as Lola indulges in her foster children’s passion for their faith, she notices their beliefs and practices become increasingly harrowing and violent. It’s a film that makes a lot of comments on the nature of religion, though this doesn’t stop the many moments that the titular evil twins in Tin & Tina terrorize Lola and Adolfo any less terrifying.

2022’s The Twin is another great horror movie with twins that sees the spirit of a deceased sibling return to seek vengeance against the one that survived. Directed by Taneli Mustonon, who also co-wrote the script, twins Elliot and Nathan are both played by child actor Tristan Ruggeri. While it’s Elliot who’s deceased, Nathan can be just as creepy in his own way. The Twin does a great job at utilizing the fact that their mother, Rachel (Teresa Palmer), is terrorized by a spirit that looks identical to her son as a source of terror, too.
However, where The Twin really shines among the best horror movies about evil twins is in the twists and turns in its plot. While there are plenty of visuals reminiscent of supernatural scary films and demonic possession stories, it is at its core a psychological horror. The ending of The Twin contains some truly jaw-dropping reveals, perfectly paying off the suspense-filled descent of Rachel as her world unravels.

Blood Rage is a textbook evil twin movie, with the 1987 slasher in many ways laying the blueprint for how the classic set-up would be approached by later films. The story sees Mark Soper as identical twins Todd and Terry Simmons, and is relatively straightforward – Todd and his mom are stalked by Terry after the evil sibling escapes a psychiatric institution. It’s undeniably simple, though this doesn’t stop Blood Rage from being an intense and thrill-filled watch.
Soper does an excellent job at portraying both Todd and Terry, distinguishing them as characters and fully leaning into the eccentricities present in both. There’s also plenty of comedy to be found in Blood Rage too, and it’s a strong example of the somewhat lighthearted slasher films of the late 1980s and early 1990s that inspired the likes of Scream. However, where Blood Rage really stands out are the practical effects present with each of the 1987 film’s many kills.

Goodnight Mommy is an Amazon Prime Video original from director Matt Sobel, and features both some of the best horror movie twins of the decade so far and some of its creepiest evil kids. The 2022 release sees actor Cameron Crovetti, known for portraying Ryan in Amazon’s The Boys, co-starring with real-life identical twin Nicholas Crovetti. The pair play Elias and Lukas, and are incredibly unsettling once the strangeness in Goodnight Mommy begins.
The plot of Goodnight Mommy sees the twins reject their unnamed mother (Naomi Watts) after she undergoes cosmetic surgery. Elias and Lukas try escaping their home, and eventually take their mother hostage. This all culminates in a jaw-dropping and surprisingly heartwrenching final twist at the end of Goodnight Mommy, and this combined with the performances of Cameron and Nicholas Crovetti make it one of the greatest horror movie about creepy twins.

Malignant is one of the most unique uses of twins in any horror movie, as the possession horror focuses on a woman possessed by the mind of her only partially formed twin brother, who exists as a tumor in her skull. The 2021 movie comes from legendary horror director James Wan, and is just as bizarre as the premise suggests. However, Malignant uses its core concept incredibly well, leading to plenty of disturbing supernatural horror visuals.
Annabelle Wallis also does a great job leading the Malignant cast as Emily, the surviving sibling. Ray Chase provides the voice of Gabriel, and the ways the parasitic twin can disrupt Emily’s life are inventive and disturbing. There are also plenty of strong psychological elements as Emily begins the realize the murders she has visions of at the start of Malignant are actually happening.

Dead Ringers stands out among the best horror movies with twins due to how effective it is as a drama, rather than relying on gore or disturbing visuals. Elliot and Beverly are both creepy and disturbing, though this is achieved without the use of any supernatural elements or practical effects. The pair, who are based on real-life twins Stewart and Cyril Marcus, are simply harrowing in their behavior and obsession with one another, and few other films in the subgenre feature identical siblings who are quite so unsettling.

A Haunting At Silver Falls is the incredibly effective supernatural horror from director Brett Donowho. Released in 2013, A Haunting At Silver Falls jumps into paranormal goings-on to introduce viewers to the spirits of murdered twins Holly and Heather (Nikita and Jade Ramsey), and the terror they can inflict when they return from the grave. The twins in A Haunting At Silver Falls are beyond terrifying, both due to the ways they’re able to torment those they haunt, and because of their appearance.
The visual effects in A Haunting At Silver Falls especially stand out among the best horror movies with kids in. The character designs for Holly and Heather are incredible, especially during moments when they appear as ghostly rotting, maggot-filled corpses. They aren’t the only ghosts in the film either, and there’s no shortage of creepy scenes as Jordan (Alix Elizabeth Gitter) attempts to unravel the mystery behind the disturbing hauntings.

When it comes to creepy twins in horror movies, none are more iconic than the Grady twins in The Shining. The 1980 film The Shining is adapted from the Stephen King novel of the same name, and includes more infamous horror movie moments than almost any other feature. Legendary director Stanley Kubrick ensured that Jack Nicholson’s Jack Torrence’s descent into madness was filled with as many unforgettable images as possible. However, while moments like the elevator of blood and The Shining quotes like “here’s Johnny” define the legacy of Stanley Kubrick’s unsettling horror, the creepy twins encountered by young Danny Torrence (Danny Lloyd) are just as well known.
Child actors Lisa and Louise Burns appeared as the ghost twins in The Shining. They stop young Danny in one of the corridors of the Overlook hotel, and creepily invite him to play with them. It’s an incredibly brief moment, but it’s one that shook many audience members to the core when the film was released in 1980. There’s something just incredibly disturbing about the Grady twins, made all the unsettling by how hard it is to explain exactly why they’re creepy. It’s hard to deny that when it comes to the best horror movies with twins in, The Shining is regarded by most as the strongest.
